There is a known issue in the mcp23017. This issue has caused that MicroChip change the specification (not the design!!), reducing the number of input pins. The issue cause the chip to malfunction and must be resetted. In my experience, the issue is mitigated largely by using debouncing capacitors, reducing the number of switches and interrupts generated.
