Hotplug USB audio

I have just pushed some fixes to the dev branch. Anyone wanting to test it out can do so. It should be fairly stable and functional but you play at your own risk. You can basically add and remove USB audio interfaces and they should appear and disappear in the audio input and output routing menus. Hotplug can be enabled/disabled in the admin menu.
